• For some, suicidal thoughts emerge and remit over time during chronic post-TBI recovery. • Across years 1–2 and 2–5, suicidal thoughts remitted in ~70% of individuals with TBI. • Suicidal thoughts emerged (6%) and persisted (28–43%) in the chronic stages of TBI.
