Exosomes, nanoscale extracellular vesicles (EVs) secreted by nearly all cell types, have emerged as promising biomarkers for cancer diagnosis due to their rich molecular cargo and inherent stability in biofluids. Raman spectroscopy and surface-enhanced Raman scattering (SERS) provide highly sensitive methods for probing the biochemical composition of exosomes, generating molecular fingerprints that enable early and accurate disease detection. This review highlights recent progress from 2020 to 2025 in the application of Raman and SERS technologies to exosome-based cancer diagnostics, with particular focus on lung, breast, colorectal, prostate, gastric, and other malignancies. Advances in nanostructured substrates, signal amplification strategies, and machine learning (ML)-assisted data analysis have significantly enhanced sensitivity, specificity, and reproducibility, enabling single-exosome detection and accurate classification of cancer subtypes. Furthermore, the development of portable Raman instruments and multimodal biosensing platforms underscores the translational potential of these methods for point-of-care diagnostics. Despite this progress, challenges remain in standardizing exosome isolation procedures, substrate fabrication, and spectral analysis, which are critical for clinical validation and regulatory approval. Looking ahead, integrating Raman and SERS with artificial intelligence (AI)-driven multimodal liquid biopsy platforms is expected to accelerate the development of clinically robust, real-time cancer diagnostic systems capable of guiding personalized therapeutic decision-making.
