• PDZ domain-containing protein 1 (PDZK1), an adaptor protein known for regulating ion transport in proximal tubules, also controls fatty acid uptake in DKD. • Mechanistically, PDZK1 directly binds FATP2, limits its trafficking to the plasma membrane, and thereby restrains fatty acid influx. • Pharmacological inhibition of FATP2 mimics the protective effect of PDZK1, alleviating tubulointerstitial injury. Targeting the PDZK1–FATP2 axis represents a precision strategy to halt or reverse DKD.
