• Remnant cholesterol is a cholesterol contained in triglyceride-rich lipoproteins after triglycerides have been removed. • We report the associations of remnant cholesterol with incidence, progression, and change in coronary artery calcium in a large cohort of South Asian Americans. • Higher remnant cholesterol levels were associated with increased odds of incident coronary artery calcium independent of traditional risk factors. • These findings suggest remnant cholesterol may be a novel risk marker and therapeutic target.
