• Human periodontitis exhibits a resolution-deficient immune landscape marked by impaired efferocytosis and reduced RvD1 biosynthesis. • FPR2 is required for RvD1-mediated periodontal protection, as demonstrated in myeloid-specific Fpr2 knockout models. • Single-cell profiling reveals that RvD1 reprograms monocyte–macrophage states toward a pro-resolving phenotype in alveolar bone. • RvD1 promotes inflammatory resolution by inhibiting the p38 MAPK–ADAM17 axis and preserving CD14-dependent efferocytosis. • RvD1 restores macrophage efferocytotic function, supporting its therapeutic potential for periodontitis and chronic inflammatory diseases.
