• Four respiratory-derived variables were evaluated for their association with labour pain intensity and diagnostic performance. • A respiratory variation index, derived from respiratory rate and amplitude variability, was strongly associated with pain severity. • The respiratory variation index may provide anaesthesiologists with an objective measure of maternal pain during labour.
