• Late-onset PJP exceeds 29% despite standard 6-month prophylaxis. • Age >45 years, peak corticosteroid dose, and lower CD4+ T cell count independently predict PJP. • PJP increases non-relapse mortality and reduces overall survival and disease-free survival. • Individualized prophylaxis strategy is needed for high-risk patients.
