• Sexual harassment is a persistent issue in operating theatres and surgical workplaces, particularly affecting women, trainees, and junior perioperative staff. • Hierarchical workplace structures and surgical culture contribute to underreporting due to fear of retaliation, career impact, and inadequate reporting systems. • Experiences of harassment are associated with psychological distress, burnout, reduced job satisfaction, and possible attrition from surgical careers. • Current evidence lacks strong intervention studies, emphasizing the need for educational initiatives, institutional policies, and cultural reforms to improve workplace safety and professionalism.
