• Integrated spatial multi-omics reveals the region-specific mechanism of Paeoniflorin in IgA nephropathy. • Paeoniflorin targets the Oat, Aco1, and Fh axis for renal metabolic reprogramming. • Paeoniflorin induces cortical TCA cycle disruption and upregulates arginine metabolism. • Tubuloimmune crosstalk between DTCs and macrophages is reshaped by Paeoniflorin. • In vitro assays confirm Paeoniflorin's direct anti-inflammatory effects via Oat, Aco1, and Fh.
