• Injectable starPEG–heparin hydrogels enable sustained MSC secretome delivery. • Secretome proteases drive controlled hydrogel remodeling and release. • Balanced cytokine and growth factor release maintained for 14 days. • Released factors retain strong immunomodulatory bioactivity. • Hydrogel delivery suppresses PBMC proliferation and shifts macrophage phenotype.
