• This study examined state-level neural dynamics that may underlie persistent bias toward negatively valenced or salient internal experiences in NSSI. • NSSI was associated with disproportionate engagement of a recurrent ventral attention-related state, reflected by greater fractional occupancy and longer lifetime. • Greater dominance of this state was associated with more severe baseline emotion-regulation difficulties and less improvement in NSSI behaviors at 3-month follow-up. • These findings suggest that altered brain-state dynamics may represent a clinically relevant mechanism in NSSI and a potential target for intervention.
