• Sucralose exposure is linked to cardiac redox-bioenergetic impairment. • Proteomics prioritizes MRPL12-linked mitoribosomal alterations. • Sucralose lowers mitochondrial translation-associated nascent protein synthesis. • MRPL12 loss phenocopies mitochondrial, redox, and lipid abnormalities. • MRPL12 restoration partly attenuates cardiac remodeling and lipid deposition.
