• We introduce the first patch-level perinatal brain age prediction paradigm. • The paradigm enables fine-grained, interpretable cortical maturation assessment. • We develop SurfAge-Net with spatial-channel mixing and lateralization-aware attention. • SurfAge-Net achieves highly accurate, efficient fine-grained brain age prediction. • SurfAge-Net provides spatially localized insights into atypical neurodevelopment.
