1. Prolonged pembrolizumab exposure after chemoradiation was associated with lower all-cause mortality in a real-world NSCLC cohort. 2. Median survival was longer with prolonged pembrolizumab exposure compared with shorter-duration exposure. 3. Prolonged pembrolizumab was not associated with increased measured hospital admission, critical care utilization, emergency care, or palliative care encounters. 4. The analysis transparently addresses immortal time bias, residual confounding, and unavailable disease-specific variables such as stage, performance status, and PD-L1 expression. 5. Findings are hypothesis-generating and support further studies using landmark analyses, time-dependent exposure modeling, and prospective treatment-duration assessment.
