Development of video capsule endoscopy progressed rapidly since it's advent almost 10 years ago, by improving existing systems, and extending the spectrum of indications. Four different companies offer video capsule systems with documentation of diagnostic and image quality. Actual developments of video capsule endoscopy include innovations in the field of hardware as improvements in optical resolution, viewing angle und battery power, bidirectional communication between capsule and receiver, and adapted frame rate. Furthermore, new software features as Progress Indicator, Flexible Intelligent Color Enhancement, and mosaic mode have been introduced. Aims of these developments are higher diagnostic sensitivity, shortening of reading times and prolongation of working time by optimizing power consumption. Better patients' acceptance might allow for colon cancer screening at home. Improvement of diagnostic sensitivity of the new colon capsule by these developments has been demonstrated in a first study, while proof of the other concepts is pending. Further innovations in the field of software are expected from preprocessing of capsule videos. Improvements in hardware include several systems for remote control of capsule movement, as well as local drug release. Feasibility and clinical relevance of these experimental approaches require further evaluation and development before application in clinical practice.
