• IRF care was associated with longer home time at 90-days and 1-year. • Gains in 90-day home time for IRF were explained by longer length of stay at SNFs. • Despite adjusting for a large number of covariates residual confounding is likely. • Future studies should avoid using home time when comparing IRF and SNF settings. • Clinical trials comparing patient outcomes between rehab settings are needed.
