• Corticobulbar motor evoked potentials (MEP) provide an objective measure of upper motor neuron dysfunction. • Prolonged MEP latencies and central motor conduction time are the most sensitive markers of corticobulbar involvement. • Incorporation of corticobulbar MEP abnormalities improves diagnostic sensitivity in Amyotrophic lateral sclerosis.
