• Examines the impact of work limiting health issues on the probability of on-the-job promotions over the lifecycle. • Uses a large panel data set from the National Longitudinal Survey of Youth 1979 (1979–2016) and county level unemployment data from the Bureau of Labor Statistics and estimates a fixed effects model. • Finds that work limiting health issues affect the probability of on-the-job promotions negatively only if an individual resides in an area with high unemployment. • The negative effect is prevalent for individuals between age 30–40 and driven by individuals not willing or able to move from counties with high unemployment.
