• L858R and TP53 independently predict worse rwPFS and OS on front-line osimertinib. • All TP53 variants, regardless of mutation type, predict inferior survival. • L858R combined with a TP53 co-mutation was associated with the shortest survival. • L858R/TP53-mutant NSCLC had a 21.3 month median OS and 5.4% 5-year OS. • EGFR subtype plus TP53 status can identify patients for intensive therapies or trials.
