The impact of pomalidomide-containing induction on stem cell mobilization and engraftment in multiple myeloma: a single-center experience

Background. Autologous stem cell transplantation (ASCT) is the standard intensification therapy for eligible newly diagnosed multiple myeloma (NDMM) patients. While lenalidomide impairs hematopoietic stem cell (HSC) mobilization, data regarding pomalidomide (POM)-containing induction remain extremely limited. Methods. This single-center, retrospective cohort study analyzed NDMM patients undergoing HSC mobilization with high-dose cyclophosphamide (CTX) plus pegylated granulocyte colony-stimulating factor between 2023 and 2025. A 1:1 propensity score matching (PSM) was performed to balance baseline characteristics. Results. From 111 patients initially included, 33 matched pairs (n = 66) were analyzed. The incidence of poor mobilization was similar between the POM and control groups (36.4% vs. 30.3%, P = 0.602). However, the POM group exhibited a marginally lower total CD34 + cell yield (median: 5.81 vs. 7.21 × 10⁶/kg, P = 0.059) and a significantly lower proportion of patients achieving the optimal target of ≥ 6 × 10⁶ cells/kg (45.5% vs. 72.7%, P = 0.024). Borderline increases in plerixafor (PXF) requirements (87.9% vs. 69.7%, P = 0.071) were observed with POM. Within the POM cohort, concomitant daratumumab (Dara) exposure significantly reduced total CD34 + yields (median: 5.19 vs. 8.24 × 10⁶/kg, P = 0.014) and increased PXF requirements (100.0% vs. 66.7%, P = 0.012). Post-ASCT engraftment kinetics were comparable (all P > 0.05). Conclusions. POM-containing induction reduces the likelihood of achieving optimal HSC collection targets and increases reliance on on-demand PXF, a negative impact further compounded by concomitant Dara use. Nonetheless, it does not preclude minimum dose collection or compromise long-term engraftment.
