The impact of SARS-CoV-2 on fatigue and quality of life in the pre-vaccination era: prevalence of post COVID-19 condition up to one year after infection in a Dutch population-based serosurveillance cohort

While previous population-based studies have assessed post-acute sequelae after coronavirus disease 2019 (COVID-19), none have investigated health-related quality of life (HRQoL) and (extreme) fatigue after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection during the pre-vaccination era. Using a nationwide seroepidemiological study, we investigated endpoints linked to post-COVID-19 condition in the general Dutch population up to one year after infection, including symptomatic and asymptomatic infections among unvaccinated individuals early in the pandemic. Participants that were not (yet) vaccinated and aged ≥ 15 years were selected from the February 2021 round of the PIENTER Corona cohort study. We assessed associations between time since serologically-identified SARS-CoV-2 infection and health utility, mental health, physical health and fatigue. To enable comparison across the full range of each outcome, we defined cut-off points for multivariable logistic regression models at every 5% increment (5–75%) along the cumulative distribution of uninfected individuals. At the lowest 15% cut-off, significant differences between uninfected (n = 4,569) and infected ≤ 4 months ago (n = 351) were observed for health utility (odds ratio [95% confidence interval]: 1.6 [1.2–2.2]), physical health (1.9 [1.5–2.5]) and fatigue (1.6 [1.3–2.1]), but not for mental health. There were no significant differences between uninfected and infected > 4–12 months ago (n = 327) for all outcomes, with post-hoc analysis showing power to detect differences down to 7%. In this population-based cohort during the first pandemic year, unvaccinated individuals infected with SARS-CoV-2 ≤ 4 months ago reported poorer health utility and physical health, and more severe fatigue compared to those uninfected. For those infected > 4 and up to 12 months ago statistically significant differences could not be detected in our analysis, suggesting a relatively low post-COVID-19 condition population prevalence up to one year after infection.
