• This systematic review provides an updated life-course account of how parental severe mental illness shapes childhood development and adult functioning. • It identified common core experiences included trauma and fear, secrecy and silencing, parentification, and limited professional and community support. • These experiences often persisted into adulthood through difficulties in relationships and self-concept, ambivalence about parenthood, and complex patterns of help-seeking, alongside evidence of resilience. • The synthesis supports identifying Adult Children of a Parent with a Mental Illness as a pragmatic nosological subgroup with distinct psychosocial exposures and clinical needs.
