• The central defect in HFrEF is impaired contractility. • AC01 improved cardiac output, stroke volume, and ejection fraction in HFrEF mice. • AC01 improved systolic function in a pre-load and after-load-independent manner. • Oral AC01 improved cardiac function and autonomic balance in nonhuman primates. • AC01 showed rapid and sustained inotropic effects across species.
