The objective of this study was to identify patients' perceptions of psychotherapeutic actions within an institutional context. A qualitative design was utilized, employing a Practice-Oriented Research approach based on Consensual Qualitative Research (CQR). Forty patients from two university-based psychological care centers in Medellin participated. Participants responded to a semi-structured interview, which explored their subjective experiences in psychotherapy. The transcription of these interviews was segmented and subsequently coded, from which three categories emerged: 1) The action woven between patient and psychotherapist (or The Relational-Action Fabric), 2) The intention of the psychotherapist's actions from the patient's perspective, and 3) What is transformed in psychotherapy. In conclusion, psychotherapy in institutional contexts is a relational process where the work woven between the psychotherapist and patient is valued by the latter as a sense-producing dialogue. In this process, each party has distinguishable functions ultimately oriented toward the patient's psychotherapeutic goals.
