• The 2026 ACC/AHA Guideline on the Management of Dyslipidemia reinstates LDL-C and non–HDL-C goals after a decade of primarily intensity/threshold-based recommendations. • Treat-to-target goals can improve clinical directionality, follow-up, and patient-clinician communication. • LDL-C, non–HDL-C, and apoB provide complementary views of atherogenic lipoprotein burden, especially in discordant metabolic states. • Goal reinstatement has implications for quality measurement, health policy, and equity in dyslipidemia care.
