• Rheumatoid arthritis (RA) significantly increases cardiovascular disease (CVD) risks. • RA-associated cardiovascular risks are driven by four interconnected mechanisms, including chronic immune-inflammatory activation; metabolic dysregulation and oxidative stress; genetic and epigenetic regulation; and the dual effects of long-term drug therapy. • Disease-modifying anti-rheumatic drugs (DMARDs) critically improve cardiovascular risks induced by RA. • The potential role of cell therapy in cardiovascular risk management induced by RA also is discussed.
