• This review summarizes the biogenesis, composition, and preparation methods of PEVs. • PEVs exert their protective effects for IBD by modulating immune and inflammatory responses, reinforcing the intestinal barrier function, and restoring the gut microbiota. • Significant barriers involving their safety, stability, and scalability need to be addressed to enable their clinical transition. • Target-ligand identification and biodistribution quantification of PEVs in treatment of IBD remain a challenge.
