• Thrombosis, recognized as an immune-inlfammatory process, involves integration of coagulation, inflammation, NET formation, complement activation and platelet-leukocyte cross talk, which is collectively termed as thrombo-inflammation • Mechanistic signatures exist across thrombotic disorders driven by unique immune-coagulant pathways such as NETosis, PF4-antibody interactions, and cytokine storm. • Emerging antithrombotic targets prioritize efficacy with reduced bleeding risk such as Factor XI/XIa inhibitors (asundexian, abelacimab), GPVI inhibitors (Revacept), RNA-based therapeutics, and NET-targeted agents (DNase, PAD4 inhibitors). • Major diagnostic advances such as fibrin-targeted PET imaging, AI-driven CTPA analysis, AI-enhanced Doppler ultrasound, microfluidic coagulation assays, and biomarker profiling using NET components and microparticles are transforming thrombosis detection. • Precision medicine is reshaping thrombosis management through multi-omics profiling, genetics and AI-based risk models, thereby enabling individualized risk stratification and personalized therapeutic approaches.
