• In this propensity score–matched comparison, TPVR and SPVR provided similar 3-year clinical outcomes in native RVOT dysfunction. • TPVR was associated with a more favorable early safety profile, with fewer major in-hospital complications and shorter ICU and hospital stays. • Pulmonary regurgitation remained comparable between groups, whereas transvalvular gradients were consistently lower after TPVR through 3 years. • These data support TPVR as an effective less invasive treatment option for anatomically suitable patients.
