• Neurodegeneration persists despite the reduction of intraocular pressure (IOP). • Longitudinal tracking in a reversible OHT model reveals neurodegenerative pathology. • IOP normalization partially rescues early function but cannot completely prevent delayed decline. • IOP-independent neurodegeneration persists after primary mechanical stress relief.
