• BCBMs are an increasing clinical challenge, driven by longer survival and improved detection of CNS involvement. • Systemic therapies with intracranial activity are reshaping BCBM management, complementing or delaying local treatments. • Targeted agents and ADCs improved outcomes in the HER2+ disease, and new options are emerging for HR+/HER2− and TNBC. • Broader inclusion of patients with CNS disease in clinical trials is essential to treatment strategies for BCBMs.
