• Treg dysfunction is a key upstream mechanism disrupting immune tolerance in bullous pemphigoid. • Treg-targeted therapies, like low-dose IL-2 and adoptive Treg transfer, can restore immune balance. • Low-dose IL-2 shows promising efficacy and safety in a recent clinical trial for BP. • A defective PD-1 pathway impairs Treg function, linking it to spontaneous and drug-induced BP. • Treg therapies offer potential for durable remission with fewer side effects than broad immunosuppression.
