• Exertional heat stress caused scrotal hyperthermia and male reproductive injury. • Menthol pretreatment reduced scrotal hyperthermia and improved testosterone. • Menthol reduced Sertoli/Leydig apoptosis and attenuated BTB protein loss. • AMTB diminished menthol protection, supporting a TRPM8-dependent mechanism.
