• Postoperative complications after abdominal surgery are frequently preceded by functional and hemodynamic alterations. • Liver and spleen elastography provide non-invasive functional imaging of early postoperative changes. • Two-dimensional shear-wave elastography enables dynamic assessment of congestion and microcirculatory disturbances. • Combined liver–spleen stiffness assessment may improve postoperative risk stratification. • Elastography remains underutilized in structured postoperative imaging follow-up.
