Unsaturated Fatty Acids Mediate the Bidirectional Link Between Psoriasis and Depression: Evidence From Observational and Mendelian Randomization Studies

• Clinically significant depression was observed in 32.7% of psoriasis patients, with disease severity and special site involvement (scalp, face, palms, soles, genitalia or joints) identified as independent risk factors. • Using multivariable Mendelian randomization adjusted for shared risk factors (BMI and smoking), we confirmed psoriasis as a causal risk factor for depression and vice versa. • Metabolomic profiling revealed that patients with comorbid psoriasis and depressive disorder exhibit significant alterations in serum metabolites, compared to those with psoriasis alone. We identified unsaturated fatty acids, particularly omega-3 polyunsaturated fatty acids (e.g., DHA), as potential mediators of this bidirectional link. • MR-based mediation analysis identified various phenotypes related to unsaturated fatty acids as potential causal intermediaries linking the two conditions. Our findings nominate metabolic interventions (e.g., omega-3 supplementation, weight management, smoking cessation) as promising strategies to simultaneously improve dermatologic and psychiatric outcomes.
