• The uric acid- HDL-cholesterol ratio (UHR) is an interesting biomarker of health risk. • Pediatric obesity might affect glucose metabolism, liver, and cardio-renal function. • UHR is associated with prediabetes, liver steatosis, cardiometabolic and renal risk. • UHR may be a simple tool for identifying obesity phenotypes at risk of comorbidities.
