• LIPI was evaluated in 1140 ES-SCLC patients from CASPIAN and IMpower133 trials. • Poor LIPI was associated with worse OS and PFS, regardless of treatment arm. • LIPI remained an independent prognostic factor in multivariate analyses. • Patients with good LIPI had longer survival with chemo-immunotherapy combinations. • LIPI may help identify ES-SCLC patients who benefit less from immunotherapy.
