• Pulmonary exacerbations produce heterogeneous changes in FVC. • Larger FVC deficits are associated with increased inflammatory burden. • Large FVC deficits identify patients at higher risk of recurrence. • Residual FEV1 deficit predicts recurrence more robustly than FVC. • FVC offers no clear advantage over FEV1 as a clinical trial endpoint.
