• In pwCF receiving treatment, BMI increased significantly during a 2-year follow-up. • Increased body weight is an independent predictor of hepatic steatosis. • Weight gain did not necessarily lead to hepatic steatosis. • Liver stiffness improves in pwCF receiving CFTR modulators. • Most pwCF with HS exhibit cardiometabolic features consistent with MASLD.
