• Target-attainment analysis identified patients with inadequate exposure requiring dose optimization. • Our Pharmacokinetic models describe substantial interindividual variability in CFTR modulator exposure. • Concomitant antifungal therapy significantly reduces clearance of elexacaftor, tezacaftor, and ivacaftor. • Higher albumin, bilirubin, and vitamins D and E are associated with modestly lower drug clearance.
